If the sponsor/donor has never donated/sponsored Midnight Sun before, please fill out the following document and email it to Paula (ppetrie@uwaterloo.ca)

If you are not sure if they have donated/sponsored us before, please check with the PM, who will either know or ask you to email Paul, who will check the system.
